When Olivia was in the hospital, Sanford talked with her about Peter, Walter and the Fringe Devision. But how could Sanford know, that Olivia had a relationship with John Scott?

It would have been in her file I guess she did move heaven and earth to safe in the Pilot and Broyles mentioned it to her as well he and Stanford were friends.

Nina knew about Olivia & John too, in the beginning of 1.02. It must have gone into Olivia's file, possibly as a reprimand for violating policy.

Since Sanford was sent by the higher-ups to give a formal review of Fringe Division, he would obviously have had access to the files of its important members which include Olivia, Peter, Walter, Charlie, and John Scott.

When I understand it right, you suppose, that Nina was the person who let wrote it in Olivias file. Do you mean this?

No, I don't think Nina added it to the file, since she's not an FBI agent. It was probably added by Broyles or by Charlie, who seemed to be her supervisor at the time. However, Nina must have had access to the file as a member of that oversight committee (that never reappeared...).
